Hi Tim,
It means 'Long may your chimney smoke' or May you have ' Long
Life.'
In the old days, if people could see that your chimney was smoking ( in the days when an open fire, coal, wood or peat was the only form of heat etc.) you were obviousy alive and well. The expression became a toast at such times as New Year.

I will use this at midnight to friends which should get some strange looks hopefully followed by impressed looks on my explanation. Then again they may just think Andy's drunk again ha ha lol
